Resumo | A hiperplasia tonsilar é uma patologia de grande incidência na faixa etária pediátrica,
e seu tratamento (tonsilectomia) é a cirurgia mais praticada por
otorrinolaringologistas. Esse procedimento é cercado de comorbidades, tendo como
principais complicações pós-operatórias: dor intensa, febre, sangramentos e
infecções. Normalmente, nenhuma substância é aplicada rotineiramente na
orofaringe com o objetivo de diminuir a ocorrência dessas complicações. Assim, os
objetivos do presente trabalho foram desenvolver um colutório à base de água de
coco em pó (ACP-411) e avaliar os efeitos de sua aplicação na orofaringe de
crianças tonsilectomizadas através da análise de parâmetros clínicos e da α-amilase
salivar. Os pacientes estudados foram divididos em três grupos de cinco pacientes
cada: sadios, hiperplasia tonsilar sem uso de ACP no pós-operatório (HT) e
hiperplasia tonsilar com uso de colutório de ACP até o 7º dia pós-operatório
(HT+ACP). Os parâmetros clínicos como nível de dor, febre, sangramento, dor de
ouvido e necessidade de uso de analgésico foram avaliados em ambos os grupos
operados (HT e HT+ACP) em dias alternados até o 7º dia pós-operatório, de acordo
com um checklist previamente elaborado. Para pesquisa da α-amilase, foram
coletadas salivas de pacientes sadios e com hiperplasia tonsilar submetidos à
tonsilectomia, com e sem o uso do ACP-411, antes e após a cirurgia. Dessa forma,
as salivas dos três grupos foram analisadas sob a forma de pools de massas
proteicas iguais com cinco pacientes em cada: sadios, HT pré-operatório, HT 1o pósoperatório, HT 7o pós-operatório, HT+ACP 1
o pós-operatório, e HT+ACP 7o pósoperatório. A α-amilase salivar foi isolada por cromatografia de afinidade com coluna
de polissacarídeo de batata e sua concentração foi mensurada por
espectrofotômetro. O experimento mostrou que os pacientes que utilizaram o
colutório ACP-411 no pós-operatório de tonsilectomias apresentaram menores
índices de dor segundo a Escala Visual Analógica de Faces, menor necessidade do
uso de analgésicos, e maiores níveis de concentração de amilase salivar.
Características do ACP-411, como baixo custo de produção, facilidade de aplicação
e provável benefício na cicatrização pós-tonsilectomia em crianças, conforme
observado neste estudo, fazem deste colutório um produto biotecnológico viável
terapêutica e mercadologicamente. |
Abstract | Tonsillar hyperplasia is a pathology of great incidence in the pediatric age group, and
its treatment (tonsillectomy) is the most commonly practiced surgery by
otorhinolaryngologists. This procedure is surrounded by comorbidities, the main
complications of which are postoperative: severe pain, fever, bleeding and infections.
Normally, no substance is routinely applied in the oropharynx in order to reduce the
occurrence of these complications in the postoperative period. The objective of the
present study was to develop a mouthwash based on powdered coconut water (ACP411) and to evaluate the effects of its application on the oropharynx of
tonsillectomized children through the analysis of clinical parameters and salivary αamylase. The patients studied were divided into three groups of five patients each:
healthy, tonsillar hyperplasia without use of ACP in the postoperative period (HT) and
tonsillar hyperplasia with use of ACP mouthwash until the 7
th postoperative day (HT +
ACP). Clinical parameters such as level of pain, fever, bleeding, earache and need
for analgesic were evaluated in both operated groups (HT and HT + ACP) on
alternate days until the 7
th postoperative day through one previously prepared
checklist. In addition, salivary fluid was collected both from healthy patients and with
tonsillar hyperplasia submitted to tonsillectomy, with or without the use of ACP-411,
before and after surgery. Thus, the saliva of the three groups were analyzed in the
form of pools of equal protein masses with five patients each: healthy, preoperative
HT, postoperative HT 1, HT 7th postoperative, HT + ACP 1st postoperative and HT +
ACP 7th postoperative. Thus, the saliva of the three groups were analyzed in the form
of pools of equal protein masses with five patients each: healthy, tonsillar hyperplasia
(HT) and tonsillar hyperplasia with use of ACP-411 (HT + ACP). Salivary α-amylase
was isolated by affinity chromatography with potato starch column and its
concentration was measured by spectrophotometer. The experiment showed that the
patients who used the ACP-411 in the post-operative tonsillectomies presented lower
pain indexes according to the Visual Analogue Scale, less need for analgesic use,
and higher levels of salivary α-amylase concentration. Characteristics of the ACP411 as low cost of production, ease of application and probable beneficial effects on
healing after tonsillectomy in children, as observed in this study, make this
mouthwash a therapeutically and marketable biotechnological product. |
